Changing Your Password
1 - Click "My Account" in the top menu bar.
2 - Select "Change Password."
Registering for an Online Account
1 - Click "Register" in the top menu bar.
If you are an Ultradent customer:
2 - Select "Create an online account and shop immediately."
3 - Verify your account with one of the following options and click "Submit":
a. Enter your account number with either a past order number or past invoice number. All of these numbers can be found on a past invoice.
b. Enter the last 4 digits of a credit card used for a previous order and the billing zip code.
4 - Select a billing and shipping address from our records. To use a billing or shipping address not on file, please call Customer Service at 888.230.1420 to have the address added to your account.
5 - Enter the remaining personal information and login information.
If you have never ordered from Ultradent before:
2 - Select "Open a new account."
3 - Complete the Business Information form.
4 - You will receive an email when your account has been approved (up to two business days).
Update Subscription Options
1 - Visit http://www.ultradent.com/newsletters/
2 - Enter your email address under "Update your member profile here."
3 - Subscribe or unsubscribe to different messages by checking the appropriate boxes.
Browsing the Store
There are several ways to browse our store:
1 - Use the menu bars on the top or left sides of the page.
2 - You may also view an alphabetical index of our products at http://www.ultradent.com/sitemap.php and navigate directly to a specific product.
3 - After adding a product to your cart, you may add additional items by entering the SKU number directly in the blank at the top of the cart page.
Delivery
Most U.S. orders placed prior to 2:00 p.m. Mountain Standard Time are processed the same day to better serve your practice. Orders are automatically shipped 3-day Express unless otherwise specified. Overnight delivery available for orders placed prior to 2:00 p.m. Mountain Standard Time.
Discounts/Promotions
Promotion codes are entered in the shopping cart below the order total. After entering the code, click "Update Cart."
Placing an Order
1 - Log in to the store.
2 - Browse the store for the desired items and click "Add to Cart."
3 - Once in the cart, you can change the quantity ordered by typing over the "1" in the Qty box.
4 - Continue shopping until your cart contains all the desired items.
5 - In your cart, select your preferred shipping method and enter a promotion code, if applicable. (To receive exclusive monthly promotion codes, subscribe to our free eNewsletter at http://www.ultradent.com/newsletters.)
5 - Click "Continue to Payment."
6 - Select a saved credit card or enter a new card.
7 - Click "Complete Payment."
Request a Catalog
To request a catalog, please call our customer service department at 800-552-5512, ext. 4100. You may also submit an online catalog request by sending an e-mail to support@ultradent.com.
Return Policy
Any items returned within 30 days of purchase will be credited 100%. Any items returned from 31-90 days of purchase willbe credited 50%. Any items returned after 91 days of purchase will not be credited.
A return authorization number must accompany all returns. To receive proper credit, please contact Customer Service at 1.800.552.5512 for assistance.

International Distribution
This Website contains products intended for lawful distribution in the USA. In certain countries outside the USA, differing legal requirements may limit the availability of certain products or provide for different product indications and claims under labeling compatible with local conditions. All products may not be available in all countries.
Sales to General Public
Four products are available for sale to the general public: Opalescence Toothpaste, Opalpix, Smilebrushes, and Ultradent Candles. Patients/consumers can purchase these products by calling our toll free number, or by visiting store.opalescence.com. With the exception of these four products, we only sell our products to licensed dental professionals.
Ultradent cannot guarantee the authenticity or efficacy of tooth whitening/bleaching products sold by an unauthorized retailer such as eBay, Smilox, DentaMart, or similar online stores. We have made efforts to discontinue selling to any dentist if we can confirm that they are the owner of an online store that sells Opalescence bleaching products. If you would like to report a Website for unauthorized sales of Ultradent's products, please complete our feedback form and select "Ultradent Overall."
Samples
We believe in our products and sample as many as possible. Please help us continue this policy by requesting only one sample per doctor per product.
Expiration Dates
All Ultradent syringes are stamped with an expiration date consisting of one letter and three numbers. The letter is a lot number used for manufacturing purposes and the three numbers are the expiration date.
The first two numbers are the month, and the third number is the last number of the year.

- X026 = Year (2006)
Latex Content
All products are latex free, with the exception of DermaDam latex rubber dam. A synthetic version of DermaDam is also available.
